Social Security's Ticket to Work Beneficiary Satisfaction Survey         
Social Security is conducting a Ticket to Work Beneficiary Satisfaction Survey. If you received a letter inviting you to participate in the survey, we encourage you to use the link or QR code provided in the letter to access the survey and to take time (only about 15 minutes!) to share your feedback.

The survey will provide valuable feedback about your experience, if any, with the Ticket to Work (Ticket) Program, including the services provided by Employment Networks (EN). Participation in this survey will help us improve the Ticket Program and ensure that Social Security is aware of the kinds of services that beneficiaries indicate they need.

Participation in this survey is voluntary and will not affect your eligibility for benefits now or in the future. Social Security will safeguard the information provided and will use it for limited purposes, consistent with applicable Federal law, regulations and directives. Social Security will not include your name or other identifying information in any published reports or presentations about the survey.
